DIRECT PAY APPOINTMENTS available:

• Superbill will be provided with payment for insurance reimbursement

Due to the complexity and variability of health insurance plans, Tree of Life Total Wellness cannot guarantee that your visit will be reimbursed by your insurance company. We recommend that all prospective patients contact their insurance provider in advance to confirm that their chosen provider is in-network and to determine whether a referral or prior authorization is required.
